What to Expect at Your Appointment

At The Green Leaf Clinic, we want your telehealth visit to be simple, comfortable, and stress-free. Here’s what you can expect when booking with us:

Booking Your Visit

Choose your appointment type (New Patient, Follow-Up, Quick Visit, or Consultation).

Payment is made securely at the time of booking. We are cash-based and do not accept insurance. You’ll receive a receipt that you may submit to your HSA/FSA if eligible .

2. Before Your Appointment

Complete your New Patient Intake Form, Medication/Supplement List, and Functional Medicine Health Checklist before your visit 

Sign the Telehealth Consent Form and HIPAA Privacy Acknowledgment online .

You’ll receive email confirmation with your appointment link, plus reminders before your visit.

3. Day of Your Appointment

  • Join your visit from a quiet, private space using a phone, tablet, or computer.

  • Make sure your device has a stable internet connection, working camera, and microphone.

  • Your provider will review your forms, listen to your concerns, and create a personalized care plan.

4. After Your Visit

  • A summary of your care plan will be provided through our secure portal.

  • Lab orders, prescriptions, or referrals will be arranged as needed.

  • You’ll receive clear next steps and, if applicable, a reminder to schedule your follow-up.

5. Cancellations & Policies

  • Please provide at least 24 hours’ notice to cancel or reschedule .

  • Missed appointments or late cancellations may be subject to a fee.

  • Telehealth is not for emergencies. In an emergency, call 911 or go to the nearest ER
